Professional Fees

You are responsible for payment at the time of your session unless prior arrangements have been made. Payment may be made by credit card, debit card, or HSA which will be kept on file and charged automatically via the Headway portal. 

 

Fee Schedule

No-Surprise Act: The No Surprise Act aims to limit the amount you pay out of pocket to a level closer to what you would pay if the healthcare provider were in-network. The Act defines this limit using a recognized market amount or qualifying figure (like the average fee for the service). You can review the details here: https://www.cms.gov/medical-bill-rights

 

Frequency of visits:

For medication management, typical visits often vary between monthly to once every 3 months. During the initial phase of treatment, you may be recommended to be seen after a few weeks only as clinically indicated.

Good faith estimates are provided below. The best estimate is the average of once every 2-month to once every 3-month visits for most patients.

 

Attendance, Punctuality, No-Show and Late-Cancelation Policy: Please attend scheduled appointments on time and notify our office in advance if you need to reschedule.


• All appointments must be canceled or rescheduled a minimum of 24 hours before the appointment. Less than 24-hour cancelation is considered “late-cancelation” and a missed appointment is considered a “no-show”. For both late-cancelation and no-show, you will be responsible for paying the full fee that insurance would pay for that session or a minimum of $150, whichever is higher. Please note that these are out-of-pocket expenses and the patient is not reimbursed by their insurance plans for late-cancelations and no-shows.


• You are responsible for coming to your session on time; if you are more than 15 minutes late, we will not be able to bill for your session and thus, it will be considered a missed appointment (no-show).


In-Network Patients

For patients with insurance plans that are in-network with HolistiCare, LLC., your costs will be based on the fee schedule mandated by your insurance provider. You can review your Headway portal to confirm that you are in-network. The out-of-pocket costs for these will vary based on your specific plan, and the specifics can be found in the Headway portal.

Good faith estimate annual cost calculation for in-network patients:

  • Monthly visits: 12 X Co-Pay/deductibles amount = Annual out-of-pocket cost
  • Every 2-month visits: 6 X Co-Pay/deductibles amount = Annual out-of-pocket cost
  • Every 3-month visits: 4 X Co-Pay/deductibles amount = Annual out-of-pocket cost

Out-of-Network Patients

For patients with out-of-network insurance or uninsured patients, you will be charged at the time of the appointment. If you have out-of-network insurance, you can submit the bill/claim with your insurance provider to be reimbursed. Please note that the reimbursement amount may not cover 100% of the charges.

Initial Appointment (60 min): $280/session

  • Follow-up appointment medication management + brief therapy (30 min): $180/session
  • Follow-up appointment medication management + therapy (60 min): $320/session

 

Good faith estimate annual cost calculation for out-of-network or uninsured/cash patients:

  • Monthly visits (out-of-network patients):
    • Initial visit: $280
    • Subsequent visits (30 min): 11 x $180= $1980
    • Total: ($280+$1980) – Reimbursement amount by your insurance

 

  • Monthly visits (Uninsured/cash):
    • Initial visit: $280
    • Subsequent visits (30 min): 11 x $180= $1980
    • Total: $2,260

 

  • Bimonthly visits (out-of-network patients):
    • Initial visit: $280
    • Subsequent visits (30 min): 5 x $180= $900
    • Total: ($280+$900) – Reimbursement amount by your insurance

 

  • Bimonthly visits (Uninsured/cash):
    • Initial visit: $280
    • Subsequent visits (30 min): 5 x $180= $900
    • Total: $1,180/year

 

  • Every 3-month visits (out-of-network patients):
    • Initial visit: $280
    • Subsequent visits (30 min): 3 x $180= $540
    • Total: ($280+$540) – Reimbursement amount by your insurance

 

  • Every 3-month visits (Uninsured/cash):
    • Initial visit: $280
    • Subsequent visits (30 min): 3 x $180= $540
    • Total: $820

Want to schedule an appointment?

Patients with H.S.A., F.S.A, or H.R.A accounts can add their account’s debit card to the Headway portal for the cost of the visit (if out-of-network or uninsurced) or co-pay/deductibles to be charged to it. You can also download the invoice of your visit from the Headway portal and submit to to your HAS/FSA/HRA plan for reimbursement.
